Answer 3. If there is a 20% variability (usually better in the
evenings) which is reproducible, this strongly suggests asthma.
The other tests mentioned are adjuncts although the use of skin
prick tests would only normally be carried out in selected circumstances
(e.g. occupational asthma). The latest BTS/SIGN guidelines do,
however, emphasise that spirometry is preferable to the measurement
of peak flow since it allows clearer identification of airflow
obstruction and the results are less dependent on effort.
